Description

This form is used if you wish to authorize another person to act on your behalf in a case with the County Governor. You can also use the form if you wish to withdraw a power of attorney that was previously granted. Private individuals who wish to grant or withdraw a power of attorney in a case with the County Governor may use this form. The form requires login with an electronic ID (e-ID).
