Complaint about healthcare

You can submit a complaint to the County Governor if you are not satisfied with the healthcare you have received. You can also complain if you have not received the healthcare you believe you are entitled to, or if other rights have not been fulfilled. By using this form, you provide us with the information we need to process your complaint. You must log in with an electronic ID (eID) to use the form. You may submit the form yourself, or someone else may submit it on your behalf. If you are submitting the form as a lawyer, you must select your organisation's actor role when you log in.
