Agreement on the Use of the Digital Funeral Notification System for Cemetery Authorities/Crematoriums

This form is for cemetery authorities and crematoriums that wish to enter into an agreement for the use of Digital Death Notification (DGM). The form is also used when a cemetery authority or crematorium wants to add a new unit to DGM or change information in an existing agreement. The County Governor is the regional cemetery authority, and the County Governor of Vestfold and Telemark has a nationwide role in matters concerning Digital Death Notification.
